RT-N66W SIP issues using public IP in LAN network

I have an Allworx PBX connected to the internet through a RT-N66W router with the latest firmware. I'm connecting using Bria. I have SIP Passthrough on and am forwarding 2088(UDP), 5060(UDP), 8081(TCP), 15000-15511(UDP), and 16384-32767(UDP) to the PBX.

With the SIP domain set to the private IP of the PBX, everything works well.
With the SIP domain set to the public IP of the network while the client is outside said network, everything works well.
With the SIP domain set to the public IP of the network while the client is inside said network, the client can receive audio, but cannot send audio.

Is there a setting I must set to ensure audio gets routed correctly? Do I have to use a different firmware? Any idea how to fix this? Thanks.
