this was working in the previous release :

AC3200 -
I had multiple ports forwarded for various accesses to my LAN from WAN.
after the install to 380.65 - they stopped working, I checked the "Virtual Server / Port Forwarding" the feature was turned on.

the list was empty.
when adding the servers back - they are accepted into the list, until which time I press apply.

the only thing I can think that I may be doing wrong; is that I am using a singular port for the "range" box,
as any range seems to be not accepted. (wrong char?)

I have not yet attempted to roll back the firmware, nor do a full NVRAM wipe; those are both on the agenda.

All my port forwards are working fine. For the Port range it's either a single value like 5151 or a range like 10051:10070

For the Local port I either do the single value or I leave it blank when doing a range cause it won't let you enter a colon there.

Thank you - that verifys my process as being correct - other than the blank for local. Attempted that as well - no dice - after a save I get this :

Check how much free nvram you have under Tools -> Sysinfo.

Sorry, been a busy few days - I unfortunately didn't remember my NVRAM amounts - I KNOW it was blinking with the yellow ! and I think it was like 52K/54K - it's been this way through the 380.65 - alphas and betas; I figured it had something to do with the 3 radios as indicated a few times.
I do NOT know which beta it broke on; as I don't use these ports on that regular basis.

but I did just factory reset and everything is great.

There you have it then. Your router is too low on nvram, there's not enough free space to write back those settings.
