Unable to save settings or browse connected clients

This is my first post after several years of using this firmware; thank you for developing and supporting it.

Recently I ran into a strange issue on RT-AC66U - any changes I'm trying to do with a web interface lead me to logging out with 'error_status=6' in the URL redirect.

This was a problem with 380.57 ; I tried to upgrade to 380.60, but the upload dialog never worked and lead to logging out.

After a couple of reboots the router has likely lost its state, and resorted to default network name and lost all settings. I was able to upload a new firmware, and restore previous settings from a backup.

However , this problem starts again - any edit I'm trying to apply (like editing dhcp entries), or even browsing the client list are guaranteed to log me out of the web interface, with the changes lost, naturally. I tried to find the web interface logs on the router as I ssh into it, but can't determine their location.

Any pointers on where to start debugging?
 
A bit of context: this does seem to be the web interface & my laptop interaction. I tried clearing cookies, different browsers etc from the laptop, and kept getting hit by this issue; using the interface from another device, a tablet, for example, works and does not log me out.
 
Same issue - I did wonder if it was the first evidence I had not "reset my settings" when it was suggested I did a year or so ago.

I first noticed it when unable to save new WOL MAC addresses

Also noticed the minus icons and device icons are not displaying on the WOL page.

Have also upgraded to 380.61 in an effort to fix.

Doesn't work using my iphone connecting via browser either... :(

EDIT - Just saved all settings and gingerly done a factory rest with screenshots of the important pages and copies of all the Open VPN Keys.

Restoring the saved settings seems to have brought everything back online including the icons that were displaying incorrectly. I can now apply changes so all working again.
