How do I configure cake QoS for FTTH?

Most FTTH services are too fast, the router needs to disable NAT acceleration to work with Cake, which for most router will cap throughput at around 300-350 Mbps max.
 
You enable it, set max up/down rates, and that's it. But as I said, if you have FTTH, your connection is probably too fast to be able to use Cake on it.
 
I have FTTH [2.5 GBit], and use FlexQoS.
