After something like 4 years on Voxel's firmware v1.0.4.46, my 2.4/5 GHz WiFi radios stopped working - no SSID and corresponding LEDs on the front are not lit. The 60 LED is lit and the antennas have lights (blue) and ethernet works fine.

I updated to v1.0.4.77, reset to factory default, and manually re-configured it to no avail. I also bounced the WiFi via the WiFi on/off button (confirmed it did something by the 60 GHz & antenna LEDs going out and back on). I then disabled 60 GHz and reset again to no avail.

Anything else I can try or is it dead?

If dead, suggestions on a replacement? I only use it as another AP for an SSID (I really should just setup an enterprise solution like Ubiquiti so have one AP with multiple SSIDs as needed). I'm using it to separate all of my security/IoT devices to their own AP (e.g., cameras, video doorbell, smart thermostat, etc.) so they aren't on the same network as most things.
 
Sorry for the late reply sometimes my R9000 acts like this, but a change of the 5ghz channel works for me, especially if I am using the 160mghz band, but yours sounds like hardware failure with the LEDs on the front that are not lit. I would get one on eBay they are now like around $100 USD !!

Yeah, I'm 99% certain the 2.4/5 GHz radios are dead. I thought about getting a replacement on ebay, but given its age (nearly 8 yo) decided to get a newer router given what I use it for.

I picked up a RAX43v2 for what I think is a decent price (and not much more than that $100). The interface is nearly identical so it was an easy setup.
