Modem? Possibly. The modem my ISP provided is supposedly part of a class action lawsuit which reads, in part:

Myself, I don't know how valid this legal crap really is. A couple years ago I was having intermittent problems that seemed somewhat related to the above so I was going to use this to leverage the ISP to swap modems.
But I got busy (which suggests it wasn't a horrible problem), the ISP doubled my service speeds (gratis, I didn't even ask), I relocated my router and I turned on QoS. I don't know if any of that had anything to do with anything but the problem subsided to the point I forgot all about it until ... I read this thread.
Maybe I'll take my own advice and see if Ping Plotter shows anything?
Every couple of seconds or so, a high-priority maintenance task runs and it winds up momentarily hogging the processor, causing latency to increase by at least 200ms and, over time, about six per cent of packets to be dropped. It affects IPv4 and IPv6 – and it spoils internet gaming and other online real-time interaction that need fast response times.
